If Jill Biden is a "doctor" why hasn't she noticed his dementia?

First, Dr. Jill knows about Joe's dementia. No question about that.
Regarding this issue about Jill Biden referring to herself as "doctor": Her use of the title has always struck me as pathetic. Her husband has a law degree. Should we call him "Dr. Joe Biden"? Should we start calling every lawyer and every other person with a doctorate degree "doctor"?
Dr. Jill wouldn't seem so special and accomplished then, would she?
